First Visit
What Will Happen On My First Visit?
Please assist us at the time of your initial visit to the office by providing the following information:
- Your x-rays, if provided by your referring doctor or general dentist.
- your health information form will allow you to provide us with your important medical history
- A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.)
Your first appointment is for information gathering. A full head and neck examination and oral cancer screening will be provided along with a record of the teeth present, any significant restorations, your gum condition and bone evaluation, an airway evaluation and photographs will be taken of your teeth. If x-rays are required, they will also be taken so that Dr. Davis may fully diagnose your oral condition.
There are times that a treatment plan is created immediately by Dr. Davis who confers with your general dentist prior to your visit. For some cases, Dr. Davis will schedule a second appointment for your consultation so that he may further discuss and develop a plan for your treatment with your dentist. This plan will include the options for your treatment that Dr. Davis and your dentist believe are best for you.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
On subsequent visits, please notify the office if you have a new medical condition, change in your medication or concern prior to each visit (e.g. artificial heart valves, hip or knee replacements or bone pins, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
